Sam’s completely out of it, but when he hears Deans strained “Sammy” he’s immediately trying to go to his rescue, never mind that he doesn’t know where up and down, left and right is right now.

Sam’s completely out of it, but Dean might need his help. And Sam might not know where left and right is, but he only needs one direction anyway: DEAN. His brothers voice is his compass.
